- Well visits with weight checks at birth, 2-4 days, 2 weeks, one month and 2 months.
- Monitoring for newborn jaundice and weight gain.
- Monitoring of maternal emotional adjustment and check-ins on post-partum depression, anxiety and pelvic floor dysfunction. Referrals provided as indicated. (provided service in newborn package even if birthing parent is not a member)
- All visits are house calls.
- Newborn screening drawn in the home.
- Guidance on breastfeeding and referral for complications such as tongue tie as indicated.
- Tele-health, house calls, and point-of-care testing as needed for urgent/sick visits included in the price of membership.

12-24 Months

- Visits at 12, 15, 18 and 24 months.

- Developmental screening and referral to early childhood developmental services when necessary.
- Screening for autism for all children at 18 and 24 months of age.

- Continuity of care for your situation with in-person visits if within reasonable driving distance (TBD – by providers discretion)
- Telehealth visits as needed for sick/urgent issues.

- Counseling for adolescent concerns such as avoidance of smoking, substance abuse and avoidance of pregnancy and STDs if needed.

This is for those who are not currently members and do not want the monthly commitment of membership. While we understand and are happy to serve you, please be aware that membership inclusion will save you money and increase your access from one appointment, to a whole month! Self-scheduling options are available upon purchase completion for this appointment.
