Tabatha Wells, MD
Dr. Tabatha Wells obtained her medical degree from the Southern Illinois University (SIU) School of Medicine and is a Board-Certified Family Physician. Dr. Wells started her Direct Primary Care practice to take back control of how she practices medicine while enhancing patient access and prioritizing the patient-physician relationship. At appointments, you can expect her to take as much time as needed to understand your health needs and ensure you feel seen and heard on every visit.
Dr. Wells sees patients of all ages, backgrounds, and levels of health. She provides a compassionate, affirming, and non-judgmental health environment, so you can trust her with any health or wellness issue you face. Newborn & 4th Trimester
What's Included With Membership
- Well visits with weight checks at birth, 2-4 days, 2 weeks, one month and 2 months.
- Monitoring for newborn jaundice and weight gain.
- Monitoring of maternal emotional adjustment and check-ins on post-partum depression, anxiety and pelvic floor dysfunction. Referrals provided as indicated. (provided service in newborn package even if birthing parent is not a member)
- All visits are house calls.
- Newborn screening drawn in the home.
- Guidance on breastfeeding and referral for complications such as tongue tie as indicated.
- Tele-health, house calls, and point-of-care testing as needed for urgent/sick visits included in the price of membership.
$150/month for Months 3-12 Of Life
3-12 Months
What's Included With Membership
- Well visits at 4mo, 6mo and 9 months and12 months.
- Tele-health, house calls, point-of-care testing and office visits as needed for urgent/sick visits included in the price of membership.
- Developmental screening.
- Monitoring of growth and nutrition.
- Guidance for what to expect at each developmental stage.
$150/month for Months 12-24 Of Life
12-24 Months
What's Included With Membership
- Visits at 12, 15, 18 and 24 months.
- Monitoring of growth and developmental milestones.
- Developmental screening and referral to early childhood developmental services when necessary.
- Screening for autism for all children at 18 and 24 months of age.

- (Can replace urgent care and still be the pediatric doc)
- Tele-health, house calls, and point-of-care testing as needed for urgent/sick visits included in the price of membership.
24 Months – 4 Years
$150/month for Month 24 to Year 4 Of Life

- Autism screening for all patients at 24 months of age.
- School/Daycare forms completed.
- Management of nutritional and developmental concerns.
- Guidance on safety, behavioral concerns and information on local infectious disease outbreaks.
4 Years to 18 Years Old
$75/month for Years 4-18 Of Life
What's Included With Membership
- Annual well child visits
- Monitoring of growth and nutritional status.
- Monitoring and management of chronic conditions such as ADHD, asthma, seasonal allergies, autism, etc.
- Mental health and academic assessment and support.
- Sports physical exams and form completion.
- Completion of FMLA paperwork as needed.
$200/month for Patients Aged 19+
19+ Year Old
What's Included With Membership
- Continuity of care for your situation with in-person visits if within reasonable driving distance (TBD – by providers discretion)
- Telehealth visits as needed for sick/urgent issues.
- Prescription of maintenance medications for ADHD, asthma, birth control, anti-depressants, allergy medications, etc.
- Counseling for adolescent concerns such as avoidance of smoking, substance abuse and avoidance of pregnancy and STDs if needed.

College Student
What's Included With Membership
- Continuity of care for your college student with in-person visits during their time in the Springfield area.
- Telehealth visits as needed for sick/urgent issues.
